Information for Motor Vehicle Accident Clients

Once you have completed and returned your motor vehicle company’s insurance forms, give us a call at 444-3600 or drop into our clinic at 69A First Street to book a physiotherapy assessment.

When you book your initial assessment, you will need to provide us with the following:

• Name of your motor vehicle insurance company
• Adjuster’s name and phone number
• Claim number
• Date of your accident

For clients with Extended Insurance Coverage:

• If you have any extended insurance coverage for physiotherapy, you will need to know your annual limit or what percentage is paid, etc.
• You must use up your extended insurance coverage first before your motor vehicle insurance kicks in. This is the procedure for all motor vehicle accident claims.
• At your last appointment of the week you pay for your treatments (and in some cases assessment) and we will give you receipts for you to submit to your extended insurance company for reimbursement.
• When you have used up your extended insurance coverage for physiotherapy we require proof of insurance exhaustion before we can start to bill your motor vehicle insurance company directly.

For clients without Extended Insurance Coverage:

• We will bill your motor vehicle insurance company directly for your treatment.
